Sounds like either an upper stack/palm key leak, or mismatched reed/mouthpiece/player combo. That or bad technique. It really isn't that hard to hit, but I have ran into people that just can't seem to "get it" for one reason or another, despite my angles on showing them, and describing it.
On some saxes it will come out very flat, which will require the venting of additional keys, as well as a different throat position to redirect the airstream, bringing the pitch up.
