I have a level air bari mouthpiece that the bite plate fell off. It had been repaired several times over the decades and was ready to fall off. There are several layers of old, dry adhesives on both the metal and plastic (rubber cement and possibly super glue). Assuming I clean of as much as the old adhesives as possible, what would be the best material to re-apply the bite plate??

IMHO....the best would be Marine-tex boat epoxy although even small amounts have gotten pretty expensive. You may be able to find the SPTW thread by EugeneHerman who fashioned a new bite plate for a BLA with the stuff.

Otherwise, any decent epoxy should work although you don't really know if the glue is decent until it fails.
